The Family Pantry Fuel Assistance Program
The Family Pantry is taking applications for Fuel Assistance beginning October 1, 2009. In order to apply you will need the following documentation:
1. Proof of identity – any one of the documents listed below

Drivers license or State id card

Work ID
|
|
MBTA card
|
|
Current USCIS employment authorization card
|
|
U.S. Passport
|
|
U.S. Military card
|
|
Student I. D. card
|
|
Certificate of U. S. citizenship
|
|
Certificate of Naturalization
|
|
Unexpired Foreign Passport with employment authorization
|
|
Alien Registration Card
|
2. Proof of address – any one of the documents listed below
|
Driver’s license if it matches the address on the application
|
|
Current lease
|
|
Current gas, telephone, electric or cable bill
|
|
2008 Mass Income tax form
|
|
Rental form with landlord’s signature
|
3. You need to provide dates of birth and social security numbers for everyone in the household (documentation not required)
4. Proof of income –documentation for each member of the household over age 18 – all documents below that apply – Each person over 18 will have to sign a form regarding their income.

Workers compensation check

Alimony & child support
|

Interest income
|

Dividends
|

Rental income
|

Cash prizes & lottery winnings
|

Estate or trust income

5. Copies of utility companies’ bills (oil, gas, electric & telephone)
6. Homeowners must provide documentation of their mortgage, annual tax bill and homeowners insurance.
7. Renters must provide landlord’s full name, mailing address and telephone number.
